WWYD (What Would You Do?)

...If one of your employees-who used to be your co-worker-all of a sudden started walking around barefoot in the office?

Would you say something to that person?  And if so, what would you say?

It's inappropriate and unprofessional.  And also, gross.  I recognize I'm not a huge fan of feet, but even still-the workplace is NOT the place to be footloose and fancy free.

I'm sure this is being done because it's thought to be quirky, or cool or something.  Whatever the reason (and I know it's premeditated with this one), it shouldn't be done.  So how do I approach it?

If they work for you now, it's your job to impress upon them that they can't walk around barefoot. Putting aside the inappropriateness, it is UNSAFE and your job is to look out for the safety of others. I'm sure it won't be easy, but way easier to confront over this than deal with worker's comp or even something less major if he/she hurts him/herself.

You just said it. Take the person aside and advise them that shoes are required in the office. Tell them it is unprofessional, unsanitary, and unsafe not to wear shoes in the office.

Be reasonable, but firm.
